What is considered best practices? To whom should the PMO report for maximum effectiveness and efficiency? CIO? CEO? Operations Manager? Other?
By PMO I assume you are referring to the Project Managemnet Officer. My first instinct would be the COO or Operations Manager. The reason for this would be that the PMO handles projects for the entire organization. Of course this just depends on the size of your organization. If Large, then most likely the COO. If medium, then perhaps the CFO or CIO. If small, then either the CEO or CFO. This issue seems to be something that requires a look at the current leadership organization and determine who would most likely be dealing with the PMO most often. If the PMO handles a lot of IT related projects then the CIO would be the most likely choice. I hope this helps point you the right direction.

PMO = Project Management Office / Officer
I don't think there is any one "right" answer, but there sure are plenty of options. In making this decision, you may want to ask yourself some questions: 1. Will the PMO handle only IT related projects or all projects? If IT only, the CIO might be your best bet ifs/he controls the IT resources as well. 2. Does your oganization have an executive, such as a Chief Administrative Officer,in charge of "shared services" or functions that serve the whole organization? If so, the group might be best organized under that area to maintain impartiality if that would be a concern in your organization 3. How does your organization prioritize its projects? Do the Project Managers get assigned to the organizations top priorities? Is there mianly consensus among the upper management on the top priorities? If so, who you organize the PMO under might be less of a concern and can be assigned to the executive or area that has the capacity/willingness or expertise to handle it.
Who PMO should be reporting to actually depends on the job function.
If the PMO is a full-time non-project oriented position, then, it should be reporting to the CEO or administrative type executive.
If the PMO is a project based position, then, depending upon what the project function relates to. For example:
IT ---> CIO
Finance ---> CFO
Operations ---> COO
